In 2012, Memorial Nursing & Rehab Center earned a 4-Star Quality rating from the Centers for Medicare & Medicaid Services (CMS).
Designed with the individual in mind, we provide our residents with the right level of care in a friendly and relaxing, home-like environment. Physician services are available 24 hours a day, seven days a week.
General and Restorative Nursing Services
Our general and restorative nursing services are designed for those who can no longer be cared for at home or in an assisted living setting due to illness, injury or other physical limitations.
We provide a full range of services that both encourage and enable residents to live a fulfilled and interesting lifestyle. These services include:

Skilled Nursing Services
Memorial Nursing & Rehab Center’s skilled nursing services benefit patients who may have suffered from a prolonged acute or chronic illness or may have had a recent illness, injury or surgical procedure. These patients are medically stable but may require an increased frequency of nursing assessment and monitoring. These services include:
- Respiratory services
- Infusion therapy
- Wound and skin care
- Pain management
- Complete diabetes treatment
- Cardiac recovery services
- On-site dietician for nutritional support
- Ostomy care
- Case management
- Discharge planning

Skilled Rehabilitation Services - Bridge to Home
Our skilled rehabilitation program is customized to meet the needs of each patient as they progress throughout their therapy stay. Our multidisciplinary team works together to contribute to each patient’s rehabilitation goals in order to increase their level of independent functioning. Our services include:
- Respiratory therapy
- Physical, occupational, and speech therapies
- Therapists on-site and employed by Memorial Health Center
- Fully equipped therapy gym
- Restorative therapy program
- Private rooms available
Cedar Lane Assisted Living Apartments
Attached to and operated by Memorial Nursing & Rehab Center, our Community Based Residental Facility (CBRF) consists of ten spacious private units designed to ensure maximum privacy and comfort. This assisted living option, provides supervised living where individuals can enjoy life to its fullest while receiving assistance with activities of daily living. The goal is to enable seniors to maintain an active, independent lifestyle.
Alzheimer’s and Other Related Dementia Care
Memorial Nursing & Rehab Center offer services and an individualized plan of care to meet the needs of those facing Alzheimer’s Disease, Dementia, or other memory challenges. We enhance our resident’s quality of life through our exceptional care and activities programming. Our Certified Activity Director coordinators a wide range of daily therapeutic and leisure activities including arts and crafts, games, reading groups, music, and church services. Our knowledgeable and experienced professionals provide sensory stimulation and one to one interaction to individualize each resident’s needs.
Our WanderGuard System helps ensure the safety of our residents.
Hospice / Palliative Care Services
In cooperation with Aspirus Comfort Care and Hospice Services, and Hope Hospice and Palliative Care Inc., Memorial Nursing & Rehab Center provide a holistic commitment to end of life care. This team approach enables our caring staff and hospice services to evaluate the right level of care for each patient. Our staff is able to provide day to day cares while the hospice staff focuses on the special needs of the terminally ill resident and their loved ones.
