Living Well with Diabetes
Memorial Health Center and Medical Center Pharmacy manage a successful diabetes education center. It offers complete diabetes services for children and adults. Instruction on all aspects of diabetes management is provided.
You may access the Diabetes Education Center through a referral from your physician or by calling the Diabetes Education Center. Services are covered by Medicare and most health insurance plans. Please ask about other coverage options.
Programs offered by Memorial Health Center – Diabetes Education Center are provided by a team of diabetes educators including registered dietitians, registered nurses, a pharmacist, and a behavioral specialist. Together they can provide you with information and guidance in selecting a diabetes management approach that best fits your lifestyle and goals.
A diagnosis of diabetes can generate a mixture of feelings, fears and questions. Programs provided offer answers to your questions, support for your concerns, and the information you will need to make appropriate choices.
Living Well with Diabetes
Knowledge about current treatment options and methods of controlling diabetes can ease adjustment to managing the disease. Memorial Health Center’s Living Well with Diabetes program meets six months out of the year to provide information and an opportunity for individuals to share feelings and experiences with others.
Meetings are open to anyone who has diabetes, family members and significant others.
